Insurance and Billing
When thinking about billing, please remember that your medical insurance policy is a contract between you and your insurance company. Not all insurance companies have the same formula for the "usual and customary" payment level, so research what your insurance company does and does not cover, and to what extent. Our fees are based on the guidelines of the American Medical Association, which takes into account the individual requirements and complexities of patient care.
Remember, any amount not covered by your insurance company is your responsibility. Payments are due within 60 days of treatment. Please contact the office with any changes in your insurance information.
If you're a part of an HMO, please be sure you have the proper referrals turned in to the office prior to your visit. Copays are required at the time of service.
Most test results are received three to five days after they're ordered, depending on location. Please call the office five days after the test for your results.
If you'd like us to release your medical records, we're required by law to get your signed authorization first. An outside service makes copies of records once a week, and that company will bill you directly for copies of your records.
Requests for prescriptions can be called into the office during normal business hours. For your own convenience, please plan ahead for weekends and holidays when it comes to your medication needs. Narcotic prescriptions will not be filled after office hours. When you call in for a refill, please have your prescription name and local pharmacy's telephone number handy. And remember that certain medications need a written prescription each time they're filled.

After regular office hours, our answering service will take your call and assist you in the case of an emergency.
